The City of Piqua has a Contractor Registration requirement for contractors who are doing work with in the City. If you have questions about the program, or have difficulty completing the application or filling out the form, please contact the Planning & Zoning Division at 937-778-2049.

*Please note that you will be asked to register for GovWell, the City of Piqua permitting software, if you are not already registered. This can be done by clicking "Sign In" in the top right of the page.
Requirements:
- Completed Contractor Registration Application [PDF]
- Bond Form (Surety Bond for $3,000)
- Certificate of Insurance w/ the City of Piqua listed as a policy holder
- OH State Contractor License (Commercial Only)
Hard Copy Submission:
To submit a hard copy to the Community Services Department please complete the PDF document linked above and provide the required materials and drop them off at the Community Services Department window on the Municipal Government Complex Second Floor with payment.
Payment:
Payment may be made as part of the online application, or may be submitted by a check made out to the City of Piqua in the proper amount.
