Applications and questionnaires to operate a temporary food service may printed by following the link below.

Applications with the appropriate fee should be returned at least one week prior to the event.

Licenses will either be mailed to you or delivered to the food service on the first day of the event.  Licenses must be posted at the food service operation.
 
This application for a license is in the *.pdf file format, which requires that Adobe Acrobat Reader be installed on your system. If you do not have Acrobat Reader, you can obtain a free copy from the Adobe web site by clicking the "Get Acrobat Reader" button below.
