| The Health Department has
statewide access and can issue birth abstracts for anyone born
in the State of Ohio. The fee is $22.00 (cash, check, or money
order) for a certified copy (only certified copies may be
attained). To receive copies by mail, please send a copy of the
application form below along with a check or money order to: |

Please include the full name at
birth, date of birth, father's name, mother's maiden name, and
your current mailing address and phone number.
Birth records may also be picked up at the Health Department
from 8:00 am to1:00 pm and from 2:00 pm to 5:00 pm Monday
through Friday.

Documents section of the City of Piqua's E-Gov Services web site.
